To begin your individualized wellness journey, click on "Individualized Care Start Here", in the menu, then click on Step 1...the yellow bar opens my clinical intake form, take your time, fill it out as accurately as you can, you'll be required to complete a three day food diary, it's most accurate if you actually document your intake over three days, and don't try to retrospectively re-create your intake include estimated serving sizes, don't try to "be good" during your recording of your intake.... you should be representing your normal intake of food. Now click "Classes and Programs", you will choose your program, once I have received your intake form and you have signed up for a plan, I will contact you to set up a time for your your first session!
